Record Keeping: Documents You Will Need to Keep

As a parent you will receive copies of multiple documents and forms as mandated by the state and federal governments. Some parents find it helpful to keep a notebook or file folder for each year their child is in school.

Listed below are documents you will want to keep in your child’s file.

  • Names and contact information of key people providing services to your child
  • Physicals and Medical Reports( including details about any medications or allergies)
  • Eligibility Conference Report
  • Specialized Evaluation Reports (speech, occupational therapy, etc.)
  • Individualized Educational Plan and associated documents
  • Explanation of Procedural Safeguards
  • Progress Reports and Goal Updates
  • Samples of schoolwork or projects
  • Correspondence to and from the school, or other service providers
  • Calendar
  • School Handbook

Keep the notebook or file where you can easily refer to it or add to it whenever needed.
